Addendum #4 to the prequalification questionnaire for the subcontractors listed below have been released and are available for download on the project web site by clicking on the title of this article.  All prequalification submittals must comply with the original questionnare and all addenda in order to be accepted by the County.  This addendum does not apply to prequalification for Design-Build teams.  This addendum applies to the following subcontractor questionnaires:

• Electrical
• Fire Protection
• Mechanical
• Metal Stud and Drywall
• Plumbing
• Structural Steel
• Healthcare Technology & IT Consultant
• Healthcare Technology Implementation & Low Voltage
